Welcome to the website and services of Ellerslie Physiotherapy & Wellness. By accessing our website or booking an appointment at our Edmonton clinic, you agree to comply with and be bound by the following terms and conditions.

1. Clinical Services & Medical Disclaimer

The information provided on this website is for educational and informational purposes only. It is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified licensed therapist or healthcare provider regarding any physical condition or injury.

2. Appointment Booking & Cancellation Policy

Your appointment time is reserved exclusively for you. Because we provide dedicated one-on-one care, we require at least 24 hours’ notice for any appointment cancellations or rescheduling.

• Cancellations made with less than 24 hours’ notice, or missed appointments (“no-shows”), may be subject to a late cancellation fee. Late cancellations or missed appointments may be subject to a fee of $70.
• Please note that insurance providers generally do not cover fees for missed or late-cancelled appointments; this balance will be the patient’s personal responsibility.

3. Payment & Direct Billing

• Payment Due: Payment for all clinic services, products, or applicable co-pays is due at the time of your treatment session.
• Direct Billing: As a courtesy, we offer direct billing to many major insurance providers, WCB, and auto insurers. However, insurance coverage is a private contract between you and your provider.
• Patient Responsibility: If your insurance claim is denied, partially covered, or delayed, you remain fully responsible for the outstanding balance of the clinic fees.

4. Patient Conduct

We are dedicated to providing a safe, respectful, and healing environment for both our patients and our medical staff. Any form of verbal abuse, harassment, discrimination, or aggressive behavior will result in the immediate termination of care and removal from the clinic premises.

5. Limitation of Liability

Ellerslie Physiotherapy & Wellness and its practitioners shall not be held liable for any indirect, incidental, or consequential damages arising out of your use of our website or clinic services, outside of the standard professional liabilities governed by our practitioners’ respective Alberta regulatory colleges.

6. Governing Law

These terms are governed by and construed in accordance with the laws of the Province of Alberta, Canada. Any disputes arising from these terms or clinic services shall be resolved within the jurisdiction of the courts in Edmonton, Alberta.
